WAIVER OF LIABILITY FOR VEHICLE TRANSPORT

Transportation of Clients

Direct care Providers are not required to transport a client or the client’s family member in the caregiver’s automobile.

If, I as a caregiver choose to transport a client, I am doing so outside the scope of my work for the agency and at my own risk of liability. RightCare Home Health Services will hold me as the caregiver personally responsible for any expense, damage or loss in which the Agency may incur as a result of the violation of this policy.

do by my signature below, release RightCare Home Health Services, and their client from any and all liability of any and all damages that may occur.
I also acknowledge that I have read the above policy transportation of clients.
